Referrals

Sevenoaks District - School age SEND

The main route for schools to access SEND support is the District Local Inclusion Forum Team (LIFT) meetings. Support at the LIFT meeting is provided by a range of professionals including the Educational Psychology Service, STLS District Lead, Special School Outreach Manager and Specialist Teachers. Schools attend LIFT meetings to gain advice and support, including schools supporting each other and share good practice.

Sevenoaks District - Early Years

The main route for schools to access SEND support is the District Early Years Local Inclusion Forum Team (LIFT) meetings. Support at the LIFT meeting is provided by a range of professionals including the STLS District Lead, Special School Outreach Manager and Specialist Teachers. Settings attend Early Years LIFT meetings to gain advice and support from a range of professionals as well as discuss requests for Specialist Teacher assessment or intervention.

Physical Disability and/or Complex Medical Need 

Referrals are taken on a case-by-case basis, usually through the LIFT or EY LIFT process.  In exceptional circumstances, where there is a student out of school or a young child who would benefit from a supported transition in their early years setting, therapists can refer directly to STLS PD following a conversation with the Professional Lead

Sensory Impairment

  • Multi-Sensory Impairment/Deafblindness​​​​​​

The STLS Sensory Service provide an open referral system for these three types of sensory impairment. Diagnostic information from Audiology and/or Ophthalmology will be required to support a referral.
