Sevenoaks District SEND/Vulnerable Pupil Transition Checklist – Primary to Secondary

This is intended to be a helpful guide for schools to enable successful transition for pupils from Year 6 to Year 7. The process starts in Year 5.

This checklist was produced by a working group of SENCOs, Head Teachers and STLS Specialist Teachers from the Sevenoaks Inclusion Steering Group, in response to the Transition Project commissioned by the North Area Kent Association of Head Teachers.

Primary schools are also asked to complete the Sevenoaks District Primary to Secondary (SEND/Vulnerable Pupils) Transition Information Sheet for children who have SEND or additional vulnerabilities before meeting with the receiving secondary school.

It is also helpful to complete a Pupil Portrait Sheet with the parent/carer to give to the secondary school, to help them to have a wider picture of the pupil.
